Participate and Become Involved

To participate in the Family Advisory Network / FAN at Cincinnati Children's Hospital Medical Center, you will need to complete and return a series of consent forms. Please download the consent forms below in portable document format (.pdf), and fill them out according to the instructions provided on each form.

• Authorization and Release for the Use and / or Disclosure of Protected Health Information for Marketing and Communications Form
• Photograph, Film or Vocal Recording Release Form
• Observer Confidentiality Agreement Form
