- Percent newly diagnosed patients with complete evaluation
- Percent newly diagnosed patients with type & extent of disease documented
- Percent of patients with adequate nutrition (Hgb) & growth (BMI & height velocity)
- Percent of patients with Crohn's Disease or Ulcerative Colitis in remission as documented in physician global assessment
- Percent patients initially receiving Infiliximab (Remicade) with documentation of (-) PPD & (+) Control or (-) CXR
- Percent patients on 6-MP, AZA or MTX whose CBC & LFT levels were monitored within the last 120 days
- Percent patients on Infiliximab (Remicade) receiving proper medication administration
- Percent patients receiving initial therapies of 6-mercaptopurine (6-MP) or Azathioprine (AZA) who were measured for TPMT levels before intial therapy
- Percent of patients intially receiving Infliximab (Remicade) with documentation of concurrent medications (6-MP, AZA or MTX)
- Percent of patients with QOL indices >180
- Percent patients / parents setting & achieving self-management goals
