    Using a maximum of one page per research project, please list all research experiences in the following outline:

    1. Type of research: clinical, basic, translational, or other.
    2. Subject matter: 3-4 sentences about your project.
    3. When, where, and for how long was the research performed: i.e., college, medical school, residency, employment, or other.
    4. Mentor(s)
    5. Indicate your role in the project design, execution, and / or interpretation of results.
    6. List any publications, describe your contribution in the writing of manuscript, and provide the citations.
    7. List any abstracts presented at local, national, or international meetings. Please attach copies of the abstracts.

    Using a maximum of one page:

    1. Indicate your ideal future career. Please give us your career goals for the short-term, the first
      3 to 5 years after fellowship, and for the long-term, up to 15 years after fellowship. Please
      indicate clearly whether or not research and academic medicine is in your career goals; if yes, to what extent?
    2. If you are unsure, comment on a mentor that you admire, i.e., whose career you find
      attractive.
