Hematology / Oncology Fellowship

Clinical Training

The primary objective during the first year of the Hematology / Oncology Fellowship at Cincinnati Children's Hospital Medical Center is to gain extensive clinical knowledge in the areas of hematology, oncology and blood and marrow transplantation (BMT). The combination of individualized learning from subspecialty attending physicians and practical experience in managing complex clinical situations allows fellows to develop proficiency and confidence in dealing with diverse clinical situations in these fields.

First-Year Fellows

During the first year, the five new fellows will rotate on service in fifteen 3.5 week blocks in: oncology -1, oncology-2 and oncology outpatient clinic (5 blocks); hematology-1, hematology-2 and hematology outpatient clinic (5 blocks), bone marrow transplant (BMT)-1 and BMT-2 (4 blocks) and miscellaneous and outpatient/elective (1 block).

Oncology-1 fellow will be the primary service fellow and cover leukemia lymphoma and solid tumor inpatients. The oncology-2 fellow will cover the ICU patients, oncology patients admitted for chemotherapy, tumor boards, sick patient visits/new referrals in clinic or ED. Oncology-2 fellow will also be the back-up oncology-1 fellow and may need to help the oncology-1 fellow as needed. The oncology clinic fellow will cover procedures, and attend specialized oncology clinics e.g. neuro-onc, late effects clinic and vascular clinic.

Hematology-1 fellow will be the primary service fellow who will cover all hematology inpatients and hematology inpatient consults. Hematology-2 fellow will cover/present all outpatient new consults, hematology case conferences, hematopathology and bone marrows and be the back-up hematology-1 fellow, as needed. The hematology clinic fellow will cover procedures, specialized hematology clinics (Sickle Cell Disease clinic, hematophilia clinic, thrombophilia clinic) and hematology case conferences.

BMT-1 fellow will be the primary service fellow covering all inpatients on the BMT floor and ICU. BMT-2 fellow will be a back up BMT fellow as needed and additionally cover bone marrow harvests, consent conferences, and BMT and immunology clinic.

During the miscellaneous/outpatient/elective rotation, fellows will be exposed to the Hematology/Immunobiology Laboratory, the Hoxworth Blood Center (blood banking), Radiation Oncology and Subspeciality clinics of their choice.

Fellows are entitled to 3 weeks vacation and one week each can be taken from oncology clinic, hematology clinic and miscellaneious / elective rotations. During each the clinical rotations, primary service fellows work closely with the attending on service on the inpatient units. Fellows will never be on call alone, but are the first on call with an attending in the respective service as backup. Calls are beeper calls taken from home.

Weekdays and Weekend Calls

Fellows will be on call on rotation such that they are on call 2-3 times a week. Calls will be evenly distributed over the year. Two weekends will be covered by first year fellows while the rest covered by senior fellows.

Senior Fellows

Second and third year fellows have no service months, providing them with protected time for basic, translational or clinical research.

Third year fellows may choose to be the 'Pretending Attending' of the month on any of the services of their choice (optional). Senior fellows will cover a maximum of one night per month on BMT. Senior fellows will cover anywhere from ½ to 1 ½ weekends a month on Hematology, Oncology or BMT service.

Fellow Outpatient Clinical Training

All fellows will be assigned a set outpatient clinic day for one day per week. First year fellows in clinic are encouraged to see new patients. In the third year, fellows will be on their regular clinic day 3 weeks per month, and rotate through specialty clinics (neuro-oncology, BMT, vascular clinic, hemophilia clinic, sickle cell clinic) on one day/week during the fourth week.

Note: This call schedule has been proposed as the call schedule for 2008-2009 year by the fellowship committee and may be subject to some changes.
