Certificate Program Application
Applications are accepted year-round for the Certificate in Medical Education at Cincinnati Children's Hospital Medical Center, and students may start in any quarter. Application materials include:
- University of Cincinnati Graduate Certificate Application – this application is completed online, printed and the submitted by mail with a $15 application fee.
Go to www.grad.uc.edu/certapp/. For Department, select "Teachers Education/Cincinnati Children's Hospital". The Program will default to "Medical Education (online certificate). On the third page of the application, select "Part-time study" and the year and quarter you wish to begin.
Please note that the start quarter you specify here is the earliest you can be registered for a course. You can start later than the quarter specified as long as you start within the same academic year (fall through summer). If you wish to delay your start until the next academic year, you will be required to reapply.
Please include a copy of the printed application with the rest of your application materials sent to the program office. - Online Certificate in Medical Education Program Application – this application is returned directly to the program coordinator with the remaining requested application materials.
- University of Cincinnati Supplemental Form – this form is in .pdf format. Please complete it, print and sign and include with your program application materials.
Additional required materials to be sent with the program application include:
- One-page essay summarizing your purpose and goals in entering the Certificate in Medical Education program, especially as related to your current position.
- Curriculum vitae
- Official transcripts from all colleges or universities attended (undergraduate and graduate)
- Two letters of recommendation from your home institution. Letters should be addressed to Dr. Raymond Baker, Program Director, at the address on the program application form, but should be sent directly to Lea Alaee.
- Photograph – black and white or color photograph suitable for a biosketch. If your home institution provides a photograph on their website, you can provide the link.

Admission Notification
Admission is offered to qualified candidates regardless of race, age, gender, ethnicity, religion and sexual orientation. The application process is non-competitive and applicants are accepted or rejected solely on the basis of academic and professional criteria. Applicants are officially notified of acceptance or denial by letter, but will also be informed via email. Interested individuals should direct inquiries to:
