Certificate Program Application
Applications are accepted year-round for the Certificate in Medical Education at Cincinnati Children's Hospital Medical Center, and students may start in any quarter. Application materials include:
- University of Cincinnati Graduate Certificate Application – this application is completed online, then printed and submitted by mail with a $15 application fee. The form and fee is to be mailed directly to the UC Graduate School.
Go to www.grad.uc.edu/certapp/. For Department, select "Teachers Education/Cincinnati Children's Hospital". The Program will default to "Medical Education (online certificate). On the third page of the application, select "Part-time study" and the year and quarter you wish to begin.
Upon submission, the program office will receive an automatic notification of your application.
Please note that the start quarter you specify here is the earliest you can be registered for a course. You can delay your start up to three quarters later (i.e. if you originally apply for a Fall start, you can delay until Winter, Spring or Summer quarter). If you wish to delay your start beyond three quarters, you will need to reapply.
- Online Certificate in Medical Education Program Application – this application is returned directly to the program coordinator with the remaining requested application materials.
- University of Cincinnati Supplemental Form – this form is in .pdf format. Please complete it, print and sign and include with your program application materials.
Additional required materials to be sent with the program application include:
- One-page essay summarizing your purpose and goals in entering the Certificate in Medical Education program, especially as related to your current position
- Curriculum vitae
- Official transcripts from all colleges or universities attended (undergraduate and graduate). Transcripts should be sent directly from the college/university to Lea Alaee.

Admission Notification
Admission is offered to qualified candidates regardless of race, age, gender, ethnicity, religion and sexual orientation. The application process is non-competitive and applicants are accepted or rejected solely on the basis of academic and professional criteria.
