Cincinnati Children’s Hospital Medical Center uses PhORCAS (Pharmacy Online Residency Centralized Application Service) for their application procedure. All documents must be submitted via PhORCAS as part of the application process.
The following items must be received in the PhORCAS system by 11:59 pm on January 2, 2013 to initiate your application:
- A letter describing your reasons for applying to this program.
- A current Curriculum Vitae.
- Copies of official transcripts from all colleges of pharmacy attended.
- Three letters of reference.
In addition to PhORCAS submission, the following items must be completed by 11:59 pm on January 2, 2013:
- A completed Cincinnati Children’s Hospital Medical Center online application for employment. The job number for this position is 60851.
- All candidates not interviewed in PPS at the 2012 ASHP Midyear Clinical meeting will also be required to complete an online preliminary interview through Cincinnati Children’s Hospital Human Resources Division. Candidates will be notified via email after complete applications are received, and this online preliminary must be completed by 11:59 on January 2, 2013.

A one day on-site interview is required to complete your application process. Interviews will be scheduled between January 18, 2013 and February 18, 2013.
Cincinnati Children’s Hospital Medical Center is a participant in the ASHP Matching Program for PGY1 pharmacy residency programs. Therefore, applicants must be enrolled in the ASHP Matching Program to be eligible for consideration. Please note: this position will not provide visa sponsorship.
