Family Advisory Council Award of Excellence in Family-Centered Care

Recognizing Health Professionals and Family Members Who Exceptionally Promote Family-Centered Care

The Family Advisory Council at Cincinnati Children's Hospital Medical Center established the "Family Advisory Council Award of Excellence in Family-Centered Care" to recognize health care professionals and family members who have been exceptional in promoting family-centered care within the Cincinnati Children's community.

How Nominees Are Recognized and Honored

Three awards will be given per year -- one staff member, one family member, and one team or department. The Family Advisory Council hosts an awards ceremony to honor the winners.

The ceremony for the Family Advisory Council Award of Excellence in Family-Centered Care will take place at 6 pm, on Thursday, November 20, 2008. (IMPORTANT: Please note the change of date.) The event will be held on the main campus of Cincinnati Children's, in the Sabin Auditorium (across the hall from the cafeteria).

Criteria by Which Nominees and Applicants Are Judged

Family-Centered Care is a way of practicing medicine that involves families as equal partners in the care of their child. The Cincinnati Children's Family Advisory Council wants to honor families, hospital staff members, and teams of people who work together, bridge gaps, and cross disciplines to deliver the best quality and experience of care.

  • Dignity and Respect: Made an effort to listen to your point of view and honor your background, values, and choices.
  • Information Sharing: Gave information that helped make the best choice for the patient.
  • Participation: Encouraged patients and families to participate in their care and decision-making at the level they choose.

Staff / Family Nomination and Team / Department Application Form Submission

The Family Advisory Council notifies nominators and nominees and provides them with details about the awards recognition ceremony. All previously nominated individuals must be resubmitted to be considered for the award.
