Get Emails for New Test Results

To sign up for email alerts whenever a new lab or test result is available in MyChart, click on Preferences in the MyChart left-hand menu. Click on Email. Select the options you want, then click on the Submit Changes button.

View Your After Visit Summaries

To view (or print) an After Visit Summary (AVS) from a previous clinic visit, click on Appointments in the left-hand menu of MyChart. Click on Past Appointments. Click on an appointment in the list to view the AVS.

Information is only available in MyChart for clinics already using our electronic medical record (check our list for details).

Request a Correction

If you see something in MyChart that you think is inaccurate or wrong, please let us know. The information you see in MyChart comes directly from our electronic medical record, and it’s very important that it’s right.

To request a correction, call Health Information Management (HIM) at 513-636-4217 or 1-800-344-2462 Monday through Friday, 8:30 am-5 pm.
