Significant Accomplishments in FY08

 Research Director Named

Jacqueline Grupp-Phelan, MD,  MPH, was  appointed the Research Director of the Division of Emergency Medicine in early 2008.  The reserch program has demonstrated increased productivity in publications, national presentations and funding.  The research focus teams include Prevention/Health Services, Clinical/Translational, Quality and Education.

Simulation and Research Center        

Training at the Simulation and Research Center has focused on preparing multispecialty teams to function efficiently in a complex health care environment.  They "practice" unusual situations and events so that clinicians are prepared to perform at their best and achieve the best outcomes in difficult situations.  Mary Patterson, MD, MEd, and Gary Geis, MD, and a team of specialists have led these efforts.

Key Areas of Research

Several key areas of develpment include work by Dr. Melinda Mahabee-Gittens where she is studying the implementation of ED Based Tobacco Prevention and Cessation projects.  This program, funded by a K23 from NCI is in the final phases of a longitudinal trial  to determine the effectiveness of an intervention to change an adolescent's smoking knowledge and behavior.  Dr. Grupp-Phelan is expanding her program of mental health screening into engagement interventions and is finishing her K23 support through NIMH.  She currently is funded through Columbia University to trial a case management approach to improve the likelihood of engagement into mental health services.  Both of these prevention efforts are unique in pediatric emergency medicine and of high priority to the health of our childfen in the future.

Corneli H, Zorc J, Majahan P, Shaw K, Holubkov R, Reeves S, Ruddy R: A multicenter randomized, controlled trial of dexamethasone for bronchiolitis. 2007; NEJM, 357:331-339

Federally funded multicenter ED RCT evaluating use of corticosteroids for infants with bronchiolitis which demonstrated no improvement in outcome or short term morbidities.

Gittelman MA, Pomerantz WJ, McNealy T: Reducing injury rates using a community based approach. 2007; J of Trauma, 63(3):S44-49

Community-based interventions offer one approach to reducing injuries; the number one cause of morbidity and mortality to persons aged 1-44 years in the United States. The Injury Free Coalition for Kids of Greater Cincinnati began its injury-reducing efforts within one Intervention Community in 2000. After reviewing injury data, engaging community members, and holding focus groups to assess community needs, specific structural and social alterations were made to engage youth in safe, supervised activities and provide them with safe play places. During the 5 year study period, the injury rate decreased significantly in the intervention community (42%) compared to the mean rate among three control sites (25.7%). Community-based interventions can reduce injuries to youth within high-risk regions of Cincinnati.

Grupp-Phelan J, Delgado S: A randomized control trial of a facilitated mental health referral for children presenting to the pediatric emergency department.  2007; BMC Emergency Medicine 7:12

This was a a randomized control trial looking at the success of a simple referral versus a facilitated referral for children with unrecognzied mental health problems to get into mental health services. The study identified the challenges of getting families back into services and identified strategies for further approaches to improving engagement in mental health services for families presenting to the Emergency Department with unrecognized mental health problems.

Timm N, Grupp-Phelan J, Kroner J: The impact of school closings on hospital staff absenteeism: A case study. 2007; J of Emergency Management, 5(6):70-72

In the event of a community-wide disaster or infectious disease outbreak, primary and secondary schools would be closed.  Little is known of the impact this would have on hospital staff absenteeism due to lack of child care.  Our study showed that short term school closings during snow emergencies did not result in significant rates of nursing absenteeism due to lack of child care.

Mahabee-Gittens EM, Gordon JS, Huang B, Slap GB: An emergency department intrvention to increase parent-child communication: A pilot study. 2007; Journal of Child and Adolescent Substance Abuse, 7(2):71-83

This project evaluated the feasibility and short term effects of a brief, ED-based parental intervention to increae parent-child tobacco comunication.  At one month follow-up, children whose parents had received the intervention were more likely to report that they would definitely not smoke in the next 6 months, and that there were increases in the number of child-initiated tobacco conversations, times parents talked to them about how to say no to smoking, and times parents talked to them about why they should not smoke.

Describes improved adherence to contoller medication (inhaled corticosteroids) prescription filling in high risk underserved children with asthma corroborating improved translation of asthma guideline to community.

Division Highlights

Pediatric Emergency Care Applied Research Network

The other area of exciting clinical research has been the continued expansion of the projects and funding of the Pediatric Emergency Care Applied Research Network (PECARN).  Twenty-five Emergency Departments with over 900,000 visits per annum, of which CCHMC is the largest contributor of patients in most of the studies are members of this network.  Under the site leadership of Dr. Richard Ruddy and Regina Taylor, CRC IV, some of the funded projects include reducing unnecessary radiation in treating abdominal and head trauma; use of corticosteroids in bronchiolitis; understanding the pediatric C-spine injury population; managing hypothermia in cardiac resuscitation; and biosignatures of infection in febrile 0-2 month olds.
