Transgender Health Program Launched

We have launched a new program to provide evidence-based, comprehensive care to this vulnerable group of adolescents in a safe and welcoming environment. Data from national studies and our Teen Health Center show that 7 to 9 percent of teens report being attracted to both or the same gender. Although some of our care providers were delivering these services in an informal manner in the past, we worked with the Office of Diversity and Inclusion to formalize these services. Lee Ann Conard, RPh, DO, MPH, is working with several divisions, including Endocrinology and Child Psychiatry, to provide comprehensive services for these patients and their families.

Transition Medicine Program Supports Young Adult Patients

As more children grow up surviving pediatric disorders, demand has grown for specialized care for these children once they become adults. Several studies have demonstrated significantly improved outcomes for young adults when they continue on pediatric protocols. In fact, Cincinnati Children’s already treats many young adults with complex medical problems from childhood. In FY 2010, our medical center experienced more than 21,000 encounters with patients ages 19 to 21 and more than 17,000 encounters with patients over 21. Our growing Transition Medicine program provides inpatient and outpatient care to youth with special health care needs, including accessible and developmentally appropriate health care, risk screening, self-management promotion and psychosocial support. Our team includes care managers, social services, a research team and three faculty members: Abigail Nye, MD, who completed a combined MED/PED residency and fellowship in Transition Medicine in Cincinnati; Jennifer Shoreman. MD, who completed a MED/PED residency and Cardiac Failure/Transplant fellowship at Indiana University; and Darcey Thornton, MD, who completed a combined MED/PED residency in Cincinnati and a fellowship in Adolescent Medicine at Texas Children’s.

Clinical trials have demonstrated that prophylactic human papillomavirus (HPV) vaccines are highly effective in preventing HPV infection, but the impact of vaccination on HPV prevalence rates in real-world, community settings is uncertain. In this study of adolescent and young adult women in Cincinnati, conducted before and 4 years after widespread HPV vaccine introduction, Dr. Kahn and colleagues found that the prevalence rate of vaccine-type HPV decreased substantially (from 31.7% to 13.4%). The decrease in vaccine-type HPV not only occurred among vaccinated young women (from 31.8% to 9.9%) but also among unvaccinated young women (from 30.2% to 15.4%). This is the first study to provide evidence of a decrease in the prevalence of vaccine-type HPV among young women and evidence of herd protection in a community after HPV vaccine introduction.

This study used the decision to initiate a specific biologic therapy as a model for understanding how adolescents participate in treatment decisions.  It addressed the roles and preferences of adolescents with chronic conditions in an actual medical decision, rather than a hypothetical issue.  

We found that adolescents participated in a variety of decision-making roles from telling parents about specific symptoms to having the final say in the decision.  Most adolescents were satisfied with how the decision was ultimately made. Our results suggest that parents and providers can assist adolescents with chronic conditions by providing them with relevant information and by helping teens develop the skills necessary for future medical decision making. 

Widdice L, Ma Y, Jonte J, Farhat S, Breland D, Shiboski S, Moscicki AB. Concordance and transmission of human papillomavirus within heterosexual couples observed over short intervals. J Infect Dis 2013;207:1286-94.
Because many human papillomavirus (HPV) infections are transient, transmission rates of genital HPV infections may be inconsistent if the interval between testing spans several months. This paper examined rates of concordance and transmission of HPV between the genitals, hands, and mouth over short intervals, (5 times over a 6-week period). The highest rates of concordance were observed soon after intercourse, suggesting some HPV DNA detection may represent contaminants from a partner, rather than an established HPV infection. Female-to-male transmission appeared more common than male-to-female transmission, which differs from other sexually transmitted viruses.
