Lung transplantation in infants with end-stage lung disease is a rarely used treatment option with only 84 infant transplantations ever performed and in only a hand full of centers across the world (2 in the last 5 years). Of the several reasons for the limited use of this treatment option, one is the assumption that the survival in infants is worse than older children. 

We looked at survival in all infants who have undergone lung transplantation in the past 25 years. The overall survival in infants was not only found to be similar to older children but in certain patients, who survive the first year of transplant or were intubated at transplant (usually a contraindication to transplantation), is much better. This report will hopefully encourage centers to use lung transplantation as an effective strategy in selected infants with end-stage lung failure.
