Transition Clinic
One of the many challenges facing young individuals with a chronic health condition is the move from seeing a pediatric (children's) doctor to seeing an adult doctor. The Transition Clinic at the William S. Rowe Division of Rheumatology at Cincinnati Children's Hospital Medical Center is set up to help young adults deal with this challenge. The Transition Clinic is held once a month in collaboration with Cincinnati Arthritis Associates. Hermine I. Brunner, MD, MSc, is the attending physician for the clinic.
Goals of the Transition Clinic are to:
- Promote continuity of care between settings
- Promote independence
- Provide emotional support during the transition period
