My Care Connection
The Cystic Fibrosis Center offers a unique information tool to help our patients and families better manage their healthcare information. My Care Connection allows you to access your child’s medical information in a secure, online format, giving you the information you need to be an active, informed member of your child’s healthcare team.
What You Can Do with My Care Connection
My Care Connection allows you to look up your child’s most current medical information:
- Lab reports (results can be viewed in text and in a graph.)
- Height and weight graph
- Lung function data (with a graph)
- Hospital admissions
- Clinic visit data
- Medications
You can also email non-urgent questions to your child’s healthcare team. Questions will be answered by a caregiver within one business day, and remain securely protected in your My Care Connection account.
Thanks to advanced password protection and nightly security assessments, you can rest assured that your child’s information is secure, and can only be accessed by you, your family and your child’s physicians.
My Care Connection is not intended to replace your personal relationships with your child’s care team. We encourage all families who use My Care Connection to contact care team members directly with major medical questions. My Care Connection is designed to enhance this relationship; through the online system, you can stay current on your child’s medical condition, making your conversations with care team members more productive and your child’s long-term care more effective.
Take a Tour and Log On
To sign up for My Care Connection, you must print and fill out a paper copy of the access form. Please send the completed form to the Division of Pulmonary Medicine office via fax or mail. When we receive the form, we will set up your account and contact you with your password. When you have your password, go to My Care Connection. Sign on using your login ID and password, and you're on your way to a more effective relationship with your child’s care team.
