Acid hypersensitivity in patients with eosinophilic oesophagitis. Scand J Gastroenterol. 2010 Mar;45(3):273-281. This is an interesting manuscript that demonstrates that the esophagus of EE patients is hypersensitive to acid exposure (as measured by time to heartburn).  Importantly, there is no differences in response to mechanical, thermal or electrical stimulation (markers of CNS sensitization), showing specificity of the response to acid. These results, combined with the recent findings that acid can increase eotaxin-3 release (Blanchard et al J Immunol; 2010;184:4033-41.), highlight the contribution of acid (reflux-associated acid or physiological acid) to EE. Based on this, the need to continue acid suppressive therapy in EE patients (regardless of GERD) should be considered.
