  • My Care Connection

    Explanation

    The My Care Connection Online Patient System is a secure website where patients, families and caregivers can view healthcare information.

    Take a tour and log on

    Take a tour of the common features shared by My Care Connection systems from throughout Cincinnati Children’s. The My Care Connection for Diabetes has been customized to reflect specific needs of children and families with diabetes.

    Access is provided through an internet site called My Care Connection (MCC). You can log on here. To log on, you must have prior authorization.

    The Benefits of My Care Connection to You and Your Family

    My Care Connection can help improve the quality of your child’s care by:

    • Providing accurate, up-to-date medical record information
    • Supporting your ability to manage your child’s condition
    • Giving you the information you need to be an active, informed member of your child’s healthcare team

    What You Can Do with My Care Connection

    With My Care Connection you can go online to look up medical information, including:

    • Lab reports (results can be viewed in text and in graph format)
    • Height and weight graph
    • HbA1c graph
    • Hospital admissions
    • Clinic visit data

    You can also email your child’s healthcare team with nonurgent questions. Questions will be answered by a caregiver within one business day, and questions and answers can be viewed only on My Care Connection. Urgent questions should be directed to the Diabetes Center, 513-636-2444  or 1-800-344-2462.

    The My Care Connection Online Patient System is not intended to replace the personal relationships you have with your caregivers. It is intended to provide an additional resource to share information. You are encouraged to communicate directly with your caregivers.

    How secure is My Care Connection?

    My Care Connection is accessed through a secure, password-protected website. Password rules are in place to ensure password confidentiality, and vulnerability assessments are completed nightly. My Care Connection follows the same privacy standards as other Cincinnati Children’s clinical electronic records.

    How Do I Sign Up for My Care Connection?

    To sign up for My Care Connection, you must complete a Request for Access form. You can complete this form during your next clinic visit, ask a member of your child’s healthcare team about getting a temporary username and password to use when enrolling online. This will give you access to My Care Connection once you complete the online enrollment process.

    You may also print and fill out a paper copy of the access form. Please send the completed form to the Diabetes Center by fax or mail (listed in the above-right portion of this screen). When we receive the form, we will set up your account and contact you with your password. When you have your password, go to My Care Connection. Sign on using your login ID and password, and you can start using My Care Connection.
