Test Cancellation Policy
Cancellation or revision of test orders must be made within 24 hours of receipt of sample. Orders for all tests scheduled for expedited processing − including all genetic pharmacology tests, STAT tests and prenatal tests − cannot be canceled or revised.
Steps for Canceling or Revising a Test Order
- Contact the Molecular Genetics Laboratory at Cincinnati Children’s Hospital Medical Center, 513-636-4474, and request a test cancellation request form. You may also download the form in portable document format (.pdf).
- Complete the form and fax it to the laboratory at 513-636-4373.
A DNA extraction fee may be assessed for processing of the sample prior to test cancellation.
After 24 hours, requests for cancellation or revision of orders will be handled on a case-by-case basis. Call the laboratory and ask to speak with the laboratory coordinator to determine the status of your patient’s sample.
