Before setting up the first appointment, our nurses work with families and referring physicians to obtain a detailed patient summary, copies of laboratory and radiology reports, pathology slides, photos and possibly radiology films. This allows us to plan the first appointment and ensure that all appropriate specialists are available to see the child that day. Our intake forms are available online for your convenience.
We advise you to contact your insurance company to verify your benefits and referral requirements. You should ask about coverage for the review of pathology and radiology imaging. These may be needed to make an accurate diagnosis.
