MyChart: Our New Portal
Kidney Transplant is no longer using My Care Connection. All families are encouraged to sign up for MyChart, the portal replacing My Care Connection.
If you’ve already registered for MyChart, you can log in to your account through the Sign in to MyPages link at the top of the page.
About MyChart
MyChart offers personalized and secure online access to portions of your child’s Cincinnati Children’s electronic medical record, Epic. Using a patient portal connected to our electronic medical record means you can view information from any clinic live on Epic, not just Nephrology.
Requesting a MyChart Account
Go to any Cincinnati Children’s clinic and ask for a MyChart activation code, no appointment necessary.
If your child is 13 or older he/she will need to sign an assent agreement to ensure he/she understands that some of his/her health information will be available to you.
