    My Care Connection is a secure information portal designed specifically for patients at Cincinnati Children’s and their families. Through My Care Connection’s secure network, you can review your child’s up-to-date medical information, learn more about his or her condition and become an active, informed member of your child’s medical care team.     What is My Care Connection?

    My Care Connection is an online service for liver transplant patients and their families at the Pediatric Liver Care Center at Cincinnati Children’s. My Care Connection provides a personalized tool that keeps you up-to-date on your child’s condition, provides convenient, secure access to your child’s medical records, and helps you better manage your child’s condition.

    Safely Access Your Child’s Information

    My Care Connection offers password-protected access to your child’s medical information. You can review updated records such as your child’s lab reports, admission records and clinic visit data. You now have 24-hour access to your child’s most current medical information, from the convenience of your home computer.

    Connect Online to Your Child’s Care Team

    While My Care Connection does not replace your personal connection with your child’s care team, it does provide a convenient source for answers to your non-urgent health care questions.

    You can send questions to the Pediatric Liver Care Center team through My Care Connection, and will receive answers within one business day. Both questions and answers are secure and confidential; they can only be viewed through the My Care Connection portal.

    Sign Up for My Care Connection

    To sign up for My Care Connection, you must print and fill out a paper copy of the access form. Send the completed form to the Pediatric Liver Care Center via fax at 513-636-5980, or by mail.
