    • College Hill Residential Psychiatry: Ask attending physician for an OTPT consult.
    • OP services at all other locations: Physician must send a written referral or electronic order for services.  Copies of the OT/PT referral can be located within the Medical Staff pages of CenterLink for both internal and community physicians. Ideally, the referral should provide the following information:
      • date
      • patient's name and date of birth
      • diagnosis
      • reason for referral
      • contact's name, address, phone number
      • referring practitioner's signature/credentials and phone/pager number
      • referring physician of record
