For Questions and Non-Physician Referrals
Contact the Division of Pulmonary Medicine at Cincinnati Children's Hospital Medical Center, at 513-636-6771 if you have questions about our services. We recommend that parents seeking a referral for their children contact their child’s primary care physician. Many insurance providers require a physician referral before authorizing us to see a child.
Physician Referrals
Physicians referring patients to the Division of Pulmonary Medicine should fax all pertinent clinical information and a written request for referral to 513-636-4615.
If additional information is needed before making a referral, physicians may call 513-636-6771 during business hours (8:30 am to 5 pm). Physicians may also contact our on-call physician through Physician Priority Link, 513-636-7997.
Location
The Division of Pulmonary Medicine is located on the Cincinnati Children's main campus, Location C (Outpatient Services Building), fifth floor.
