Performance Goals
For practices participating in the Collaborative, here are the graduation criteria. Once eligible for graduation, offices are welcome to continue to access the ADHD Consult Line and to attend educational events and Quarterly Meetings.
For primary care providers not participating in the Collaborative, these measures are a set of office process and patient outcome thresholds that demonstrate successful incorporation of the American Academy of Pediatrics (AAP) and Cincinnati Children's Hospital Medical Center guidelines into office-wide management of patients with ADHD.

- Completion of Initial Parent Vanderbilt Rating Scales Prior to Diagnosis
- Completion of Teacher Initial Vanderbilt Rating Scales Prior to Diagnosis
- Patients Diagnosed with ADHD Meet DSM Criteria on the Rating Scales
- Percent of Patients with a Written Care Management Plan within 14 days of Diagnosis
- Patients Have Telephone or Office Follow-Up within 14 Days of Medication Initiation
- Patients Have an Office Visit within 6 Weeks of Medication Initiation
- Completion of Parent Follow-Up Vanderbilt Forms within 6 Weeks of Medication Initiation
- Completion of Teacher Follow-Up Vanderbilt Forms within 6 Weeks of Medication Initiation
