Why Refer Patients to Us?
When you refer patients to the Division of Behavioral Medicine and Clinical Psychology at Cincinnati Children's Hospital Medical Center, you're putting them in the skillful hands of pediatric psychologists who provide children and families with the highest-quality health care. Our team approach combines state-of-the-art assessment procedures and empirically based methods of intervention and / or treatment to help every patient and family.

How to Refer
Referrals to the Division of Behavioral Medicine and Clinical Psychology team at Cincinnati Children's can be made one of two ways:
- A physician may contact us with a referral
- The family may contact the division
Parent / Family Referral
Parents / families may contact the Division of Behavioral Medicine and Clinical Psychology directly to initiate a referral. Simply call our intake line, 513-636-8107. Before you call, please have the following items in front of you:
- Child's Social Security Number
- Insurance card
If your child requires neuropsychological testing, a referral to the Psychology team needs to be initiated by the child's primary care physician. Neuropsychological testing is used for children with documented neurological conditions, such as:
- Head injury
- Seizures
- Brain tumors
Physician Referral
Patients are referred to the Division of Behavioral Medicine and Clinical Psychology at Cincinnati Children's using the referral form on this page. This form may be faxed to 513-803-1111 or toll free at 866-877-8905.
Once the referral is received by fax and processed, we will send a letter back letting you know the disposition of the case.

What to Bring to Your First Visit
At the initial visit, the parents / caregivers of the child meet with the psychologist alone.
Families may bring their child's test results and medical records with them to the first appointment. Families should also print and complete the downloadable forms below.
