Before making an appointment with the Hemangioma and Vascular Malformation Program at Cincinnati Children's Hospital Medical Center, it is best (but not necessary) to have a physician referral. To make a referral, families and physicians should call 513-636-7742. After 4:30 pm, please leave a message and your call will be answered the next business day. You may also send us an email at hvmc@cchmc.org.
Prior to setting up the first appointment, our nurses work with families and referring physicians to obtain a detailed patient summary, copies of laboratory and radiology reports, bone marrow slides, photos and possibly radiology films. This allows us to plan the first appointment and ensure that all appropriate specialists are available to see the child that day. Our intake forms are available online for your convenience.
Our social worker can work with families traveling from out of town to help them make arrangements for lodging and transportation.
Several Specialists, a Single Hospital Visit
One of the benefits of our program is that children can be examined by several specialists in a single hospital visit. While this is more convenient than making several trips to the hospital, co-payment requirements remain the same. For example, if your insurance provider requires you to make a $10 co-pay per physician visit and your child is seen by three specialists, the total amount owed would be $30.
Co-payments are due when services are provided to your child. We will not know how many physician appointments your child will have until the time of the visit, so please be prepared to make as many as six co-payments.
In addition, we advise you to contact your insurance company to verify your benefits and referral requirements. Your insurance company may not cover certain expenses, such as deductibles, out-of-pocket costs and charges above the reasonable and customary allowance. In addition, you may need to obtain referrals for each physician visit. Please check with your insurance company.
