Your First Visit
Thank you for choosing Cincinnati Children's Hospital Medical Center for your child’s care. Your first visit is a good opportunity for our team of specialists to get to know your family, better, understand your child’s medical history and begin developing the best treatment plan possible. While here, you and your family will be treated with respect and compassion. We encourage you to ask questions and share information to help us best meet your child’s needs.
How to Prepare
Our care team would like to learn as much about your child’s medical background as possible prior to your first visit. Please complete the medical history forms provided to you and bring them to your first appointment. These forms, along with any records your referring physician sends to our team, will help us make the most of our time together. Be sure to include information about prescriptions, over-the-counter medications, vitamins and herbal and nutritional supplements your child may be taking.
