Survivorship Facts
- One in 300 Americans are diagnosed with cancer before the age of 20.
- Nearly 80% of those patients go on to become long-term survivors.
- Today survivors of childhood cancer can be expected to account for one in 450 young adults.
- Cancer survivors have unique medical problems, requiring specialized care.
- Almost two-thirds of survivors will have at least one chronic health condition because of their cancer or its treatment.

Why should I schedule an appointment?
The Long-Term Follow-Up Program provides a review of your previous cancer therapy. We will work with you in healthy lifestyle management and preventive medicine initiatives. Our main focus is to keep you as healthy as possible.
Who do we serve?
- Patients (children and adults), who received cancer treatment for a pediatric cancer who are five or more years from diagnosis and at least two years off therapy
- Patients who received treatment at Cincinnati Children's or at an outside hospital
- Adult patients who are survivors of childhood cancer
- Community physicians looking for an educational resource

- An annual comprehensive visit including risk-based screenings for potential late effects:
- Preventive medicine
- Counseling and appropriate screenings

- Healthy weight loss and food choices and physical activity
- Infertility risk assessment
- Personalized treatment summaries
- Family cancer risk assessment
- Opportunities to participate in novel research studies
