What is My Care Connection?
The Cystic Fibrosis / CF care team at Cincinnati Children's Hospital Medical Center is excited about sharing a new information tool with you.
My Care Connection (MCC) is a secure web site where patients, families and caregivers can view key health care information using a computer.

Access is provided through an internet site called My Care Connection. You can log on here. In order to log on, you MUST have authorization.

What is the Purpose of My Care Connection?
The purpose of My Care Connection is to improve the quality of patient care. It includes accurate, up-to-date medical record information. The portal supports your ability to manage your child's condition. It provides information you need to be an active, informed member of our health care team.
The portal includes:
- Lab results (Results can be viewed in text and in a graph)
- Height and weight data (with a graph)
- Lung function data (with a graph)
- Recent clinic visit dates
- An area where you can ask non-urgent questions to specific members of the Cystic Fibrosis Care team. Your question will be answered by a caregiver within three business days.
Both questions and answers can only be viewed on the portal. If you have a communication requiring more urgent attention, call your Cystic Fibrosis nurse or physician or contact the pulmonary physician on call at 513-636-4200 or long distance at 800-344-2462.
Because we want to provide you with the best information on your child's care, My Care Connection is continuously being updated and expanded. We're working on new sections on medications, immunizations and test results.
My Care Connection is not intended to replace the personal relationships you have with your caregivers. It is intended to provide an additional resource to share information. You are still encouraged to communicate directly with your caregivers.

How Secure is My Care Connection?
My Care Connection is accessed through a secure, password-protected web site. Password rules are in place to ensure password confidentiality. Vulnerability assessments are completed nightly. The portal follows the same privacy standards as other Cincinnati Children's clinical electronic records.

How Do I Sign Up for My Care Connection?
An authorization form must be completed before access to My Care Connection is granted. Please print the authorization form (in .pdf format), fill it out completely and send it to the Cystic Fibrosis office via fax or mail (listed below). When we receive the form, we will set up your account and contact you with your password. When you have your password, go to My Care Connection and sign on using your login ID and password.
