As your Diabetes Center care team at Cincinnati Children's Hospital Medical Center, we are excited about sharing a new information tool with you. The My Care Connection Online Patient System is a secure web site where patients, families and caregivers can view key health care information using a computer.

Take a tour of the common features shared by My Care Connection systems from Cincinnati Children's. The My Care Connection for Diabetes has been customized to reflect specific clinic needs.
Access is provided through an internet site called My Care Connection (MCC). You can log on here. In order to log on, you MUST have authorization.

What is the purpose of My Care Connection?
The purpose of My Care Connection is to improve the quality of patient care. It includes accurate, up-to-date medical record information. My Care Connection supports your ability to manage your child's condition. It provides information you need to be an active, informed member of our health care team.
My Care Connection includes the following medical information:
- Lab reports (Results can be viewed in text and in a graph.)
- Height and Weight Graph
- HbA1c Graph
- Hospital admissions
- Clinic visit data
Another purpose of My Care Connection is to improve communication. There is a Question and Answer section where you can ask non-urgent questions. Your question will be answered by a caregiver within one business day. Both questions and answers can only be viewed on My Care Connection. Any urgent question should be directed to Diabetes Center, 513-636-2444, 1-800-344-2462.
The My Care Connection Online Patient System is not intended to replace the personal relationships you have with your caregivers. It is intended to provide an additional resource to share information. You are still encouraged to communicate directly with your caregivers.

How secure is My Care Connection?
My Care Connection is accessed through a secure, password-protected web site. Password rules are in place to ensure password confidentiality. Vulnerability assessments are completed nightly. My Care Connection follows the same privacy standards as other Cincinnati Children's clinical electronic records.

How Do I Sign Up for My Care Connection?
An authorization form must be completed before access to My Care Connection is granted. Please print the authorization form (in .pdf format), fill it out completely and send it to the Diabetes Center office via fax or mail (listed below). When we receive the form, we will set up your account and contact you with your password. When you have your password, go to My Care Connection. Sign on using your login ID and password, and you're on your way.

You must have Adobe Acrobat" Reader installed on your computer to read this file. You can download Adobe Acrobat" Reader at Adobe's Web site by selecting the version appropriate for your type of computer.
