TrYAD Clinic: Transition of Youth and Young Adults with Intellectual Disabilities
TrYAD is a multidisciplinary clinical service dedicated to serving the unique transition needs of youth and young adults with intellectual disabilities. TrYAD promotes self-management, advocacy, and continuity of care through a graduated transition process which involves:
- Education of patients, families, and their health care providers
- Coordination of ancillary services
- Liaison with community resources

The TrYAD Clinic provides specialty care. The goal of TrYAD is to work with the primary care physician(s) during this time of transition, not to replace them. We are also available for one-time consultations for families and physicians who need some assistance or would like further education about this process.
Referral population:
Our clinic sees individuals 15 to 24 years old with intellectual disabilities (mild to severe) of genetic or unknown etiology. This includes individuals with autism spectrum disorders who have intellectual disabilities. Supplementary documentation of need(s) is encouraged (for example: most recent testing, school reports, IEP).
We commonly see teenagers and young adults with:
- Fragile X syndrome
- Klinefelter’s syndrome
- Prader-Willi syndrome
- Turner syndrome
- Velo-cardio-facial syndrome (VCFS)
- Williams syndrome
Services for patients and families include:
- Patient and family education on health and wellness issues particular to teenagers and young adults
- Behavior management, including medication management for behavior
- Assistance in development of life skills and self-care abilities
- Facilitation of transfer of care into the adult health care system
- Educational counseling and coordination of information exchange between schools and the medical team
- Assistance with vocational rehabilitation services, including eligibility information and referrals to community agencies
- Facilitation of connections to community services for recreation and social skills development
- Counseling in financial resources, including health insurance and waivers
- Discussion of guardianship and estate planning
