The Cytogenetics Laboratory at Cincinnati Children's Hospital Medical Center provides sample requirements and shipping instructions for CGH microarray testing. Please note that two different tubes are needed:

3 cc whole blood in purple top (EDTA) tube AND
3 cc whole blood in a green top (Na Heparin) tube

Sample Requirements

Amount Needed	Two tubes of 3 cc each
Quality	Whole blood
Collection / Transport Container	(1) EDTA AND (1) Sodium heparin (green top) tube placed in a sealed plastic biohazard transport bag.
Specimen Labeling	Patient full name; date of birth; diagnosis. All specimen containers must be clearly labeled with the patient's full name.
Storage Conditions	Room temperature; may be refrigerated if overnight
Transport Temperature	Room temperature

When sending samples to the Cytogenetics Laboratory, please follow instructions below. Contact the laboratory with any questions.

Shipping Instructions

All specimen containers must be labeled clearly with patient's full name and date and time of specimen collection.
Please enclose the appropriate test requisition with sample. All information must be completed before sample can be processed.
All specimens should be transported promptly to the laboratory at room temperature in leak–proof containers.
Samples are delivered by couriers between 10:00 am - 12 midnight.
After hours and weekend samples should be delivered to Children's Hospital Laboratory Processing, 4th floor, Hospital Tower.
It is the policy of the laboratory to accept and set up all specimens regardless of sample size or condition.
If the specimen is judged to be less than optimum, the physician will be notified immediately by phone or FAX that the turn around time may be increased.
