The Hereditary Cancer Program at Cincinnati Children's Hospital Medical Center provides some guidelines on how to refer a patient to our program.
You or your patient may call 513-636-4760 and select option 1 to schedule an appointment.
Or, a referral form my be faxed to 513-636-4797 and the patient will be contacted to schedule an appointment.
Clinic times include Monday morning, Wednesday morning and Thursday afternoon at the main hospital location on Burnet Avenue. Appointments are also available at select contracted centers in the Greater Cincinnati area.
