Children facing a serious illness want to be like other children. They want to interact with their friends, surrounded by their treasures, in the security of their own home.

The Home Care team from Cincinnati Children's Hospital Medical Center offers "Transitions," a special service that provides the best care for seriously ill children in the most comfortable setting at home.

Transitions is a skilled professional service, ordered by a physician, for children living with a life-threatening, chronic, or debilitating condition. These special children and their families face decisions regarding treatment options, goals and interventions that pose significant physical, social, emotional and spiritual challenges.

Our goal is to enhance the care that you child is currently receiving by supplementing those services in the home. Transitions offers children and their families assistance with the multitude of challenges they routinely face, such as pain and symptom management, medical decisions and treatment options, adaptive equipment needs, school intervention, stress management, financial burdens and spiritual crises.

Our pediatric specialists work as a team to bring their expertise to each child in their care. A nurse, working with the child's attending physician, coordinates services, which range from nursing to home health aides, social services to spiritual care, occupational and physical therapy to massage therapy.
