Health care providers can refer families to our clinic by providing the central number: 513-636-2326. Communication with the primary care provider of both mother and infant is a top priority. With the mother’s permission, a copy of the visit note will be faxed to the primary providers of both mother and infant within 2-3 days of the visit.
