What is the purpose of the Liver Care Portal?
The Liver Care Portal allows families to have access to accurate, up-to-date medical data. The family can choose to share this information with health care providers to make medical decisions. The Liver Care Portal, accessible through a password-protected site, provides access to critical information including:
- Lab reports (Results can be viewed in text and in a graph, making the results easier to interpret)
- A complete list of medications and allergies (which improves patient safety)
- An ongoing list of clinical concerns
- Hospital admissions and annual visit clinical data
The Liver Care Portal also allows each patient (or family member) to submit a question electronically to the liver care team; questions and answers can be viewed online. Families can also notify us of changes in address, phone numbers, insurance, etc., through the portal.
