Test Cancellation Policy
Cancellation or revision of test orders must be must be made within 24 hours of receipt of sample. Orders for all tests scheduled for expedited processing -- including all genetic pharmacology tests, STAT tests, and prenatal tests -- cannot be cancelled or revised.
Steps for Cancelling or Revising a Test Order
- Please contact the Molecular Genetics Laboratory at Cincinnati Children's Hospital Medical Center, 513-636-4474 and request a Test Cancellation Request Form.
- Please complete the form and fax it back to the laboratory at 513-636-4373.
- A DNA extraction fee may be assessed for processing of the sample prior to test cancellation.
After 24 hours, requests for cancellation or revision of orders will be handled on a case-by-case basis. Please call the laboratory and ask to speak with the laboratory coordinator to determine the status of your patient's sample.
