Test Ordering Procedure
To order a test from the Hereditary Liver Diseases Genetics Testing Service at Cincinnati Children's Hospital Medical Center, please follow the following steps:
- Download and complete a Heritable Liver Diseases Testing Requisition.
- Place at least 2 mls of whole blood in a purple-top (EDTA) tube. Label tube with patient’s name, birth date, and date of collection. Phlebotomist must initial tube to verify patient’s identity.
- Ship specimens via overnight courier to:

Billing and Reimbursement
Most insurance plans reimburse for genetic testing. Many insurance carriers do mandate pre-authorization for testing performed by out-of-network laboratories. It is important that your patients follow the steps outlined by their insurance carriers in order to secure reimbursement for genetic testing in our laboratory. Patients are responsible for all copays and deductibles.
We will bill commercial insurance and accept most medical assistance programs, if you provide us with complete billing information on the test requisition.
