The Molecular Genetics Laboratory at Cincinnati Children's Hospital Medical Center performs a variety of tests for patients of all ages. Doctors need not be affiliated with Cincinnati Children's to order tests.

Customs Forms
Physicians from outside the United States who are requesting tests will need to complete a customs form to accompany their samples. You can view a sample of how to complete the form by downloading this 1.4 MB .pdf file.
