If you would like to request an appointment with the Comprehensive Neuromuscular Care Initiative at Cincinnati Children's Hospital Medical Center, please follow these steps:
Step 1: Fill out the Neuromuscular Intake Form.
The form is available as a word document and also as a .pdf file.
Step 2: Send the form to neuromuscularcare@cchmc.org.
Step 3: You will be contacted to schedule your appointment.

The Comprehensive Neuromuscular Care Initiative is located on the main campus of Cincinnati Children's in Location A, level 7.
