Information For Professionals
The Cincinnati Children’s Hearing Aid Trust’s vision is to offer the gift of hearing to as many Ohio children as possible. CCHAT recognizes the importance of offering early hearing intervention to children with confirmed hearing losses and the fiscal and other challenges associated with obtaining hearing aids in a timely fashion. The mission of the Cincinnati Children’s Hearing Aid Trust is to provide these children, from birth to 3 years of age, in the stage of Ohio with their first set of hearing aid(s) for free. Before CCHAT will supply funding for its applicants, each patient is required to apply for alternative coverage through both BCMH and Medicaid.
When a child is diagnosed with hearing loss, their BCMH / Medicaid approved audiologist will contact CCHAT. CCHAT will facilitate shipment of a hearing aid(s) directly to the practicing audiologist. Practicing audiologist and physician must be BCMH / Medicaid approved. CCHAT has partnered with Phonak and Oticon to provide patients with hearing aids for a three-month loaner period. During the loaner period, CCHAT will help patient families apply for financial reimbursement from a variety of government agencies. If funding is denied, CCHAT will be the payer of last resort for your child’s hearing aid(s). CCHAT will also help you develop a savings plan for future needs.
New Applicants
If you have a patient who is interested in seeking financial assistance through the Cincinnati Children's Hearing Aid Trust, please submit a completed Provider Application, if a new provider, as well as a completed Hearing Aid Request form. You will need the following information for form completion:
- The most current Audiogram; taken within the last six months
- Other tests that may be needed for diagnosis
- Physician visit forms if applicable
- Are you BCMH and Medicaid approved?
- If patient is a pre-existing Medicaid patient, please include copy of Medicaid Card
- Your Ohio license number
Additional Forms to be submitted by the practicing Audiologist:
- Once the hearing aid order has been placed, CCHAT Coordinator may contact dispensing audiologist for completion of additional forms needed for CCHAT applicant.
After Form Submission:
- Please instruct your patient’s guardian to visit our website, (www.cincinnatichildrens.org/cchat), under Patient Education, to complete application process and for step by step instructions. Hearing Device will not be ordered until guardian contacts CCHAT Coordinator.
- After guardian has contacted CCHAT coordinator, the next steps for application process will be discussed and hearing aid order will be placed.
- Hearing device will be sent to the practice listed within 48 hours after guardian has contacted CCHAT Coordinator.
- Audiologist will be reimbursed at time of hearing aid funding.
