How to Make an Appointment for Your Child

The Occupational and Physical Therapy Department  at Cincinnati Children's Hospital Medical Center requires all children to have a physician's referral for evaluation and treatment. This referral is required for treatment at any of the outpatient facilities as well as the base location at Cincinnati Children's Burnet campus.

After you have received your physician's referral, call 513-636-4651 to make an appointment.

Individual Needs Assessment

When your child is reffered to the Occupational Physical Therapy Department at Cincinnati Chidren's, your child will undergo an evaluation. At the conculsion of your child's evaluation, if therapy is recommended, you will be given an Individual Needs Assessment questionnaire.

This questionnaire will help us identify patient and family goals and concerns. This assessement will ask questions about any issues with pain, preferred language and learning needs of your family as well as any cultural or spiritual needs your family would like to have considered in planning for therapy.

Attendance

The Occupational Therapy and Physical Therapy Department at Cincinnati Children's is committed to you and your family. We want all patients to make progress and meet therapy goals. Your child's success is dependent upon prompt and consistent attendance in therapy. A signed copy of our Attendance Expectations must be returned to your child's therapist at their second treatment appointment.
