| "I really feel great when I come here. Sometimes I don't want to come but then when I come I just can't resist. I play games. ... It helps me have confidence. ... The people here are really, really nice, and they're always there to care for you just like your mom or dad. So I think this is a great place for people with disabilities, just like me." - Alex Ryan, 8 years old |
This questionnaire will help us identify patient and family goals and concerns. This assessement will ask questions about any issues with pain, preferred language and learning needs of your family as well as any cultural or spiritual needs your family would like to have considered in planning for therapy.
