ID Number
This is slide 5 out of 13.
Upon admission for confidentiality and the protection of your child, you will be given a four-digit ID number. This number is required for visiting, taking your child on off-site passes, and phone calls. You are responsible for deciding who visits, contacts and receives information regarding your child.
If you lose the four-digit ID number, staff may ask you for proof of identification. Identification is needed before the ID number can be given out again. These steps are designed to keep your child safe and ensure the confidential nature of your child's admission.
