Appointments and Referrals to the Skeletal Dysplasia Center
The Skeletal Dysplasia Center at Cincinnati Children’s Hospital accepts both written referrals from physicians as well as self referrals from patients.
Physician Referrals
Physicians can provide written referrals to the Skeletal Dysplasia Center by downloading the referral form below. Often, someone from the Skeletal Dysplasia Center will call the referring physician to gain a better understanding of each patient’s features and reasons for the referral. This will help the clinicians prepare for each patient. Referrals from primary care physicians are also useful because they allow individuals from the Skeletal Dysplasia Center to communicate with the physicians that manage the patients more frequently.
When providing a written referral please include the following information:
- Referring physician’s name, address, and phone number
- Patient information
- Reason for referral

Self-Referrals
The Skeletal Dysplasia Center at Cincinnati Children’s Hospital Medical Center is happy to see patients that make a self-referral. If you are interested in making an appointment, but are not being referred by another physician, please call our center (513) 803-2663. One of our clinical coordinators will speak with you about your reasons for wanting to be seen in the Skeletal Dysplasia Center. It is important that individuals requesting appointments be prepared to get their medical records and bring them to their appointments. This will provide our clinicians with more information about the patient which will help them provide them the most reasonable care and management.
