Overview
The Rehabilitation and Transitional Care Department at Cincinnati Children's Hospital Medical Center is comprised of two distinct programs to better provide care for patients.
Inpatient Pediatric Rehabilitation Program
The Inpatient Pediatric Rehabilitation Program is dedicated to meeting the comprehensive rehabilitation needs of infants, children and adolescents.
Working together as a team with patients and families, the goal of the program is to minimize the impact of activity limitations and participation restrictions on children and to maximize their participation in daily activities as they return to home and the community.
Patients who are cared for in the Inpatient Pediatric Rehabilitation Program include children who have:
- Congenital or acquired impaired functional abilities as a result of brain injury
- Spinal cord injury
- Neurological, neuromuscular, rheumatological or other physiological impairments
Transitional Care Center
The Transitional Care Center is designed to provide comprehensive, coordinated care to medically complex pulmonary patients and their families.
The primary goal of the Transitional Care Center is to achieve medical stabilization, provide family support, education and attainment of resources to enable patients and families to successfully manage their care at home.
Patients who are cared for in the Transitional Care Center include children of all ages who have pulmonary conditions that require tracheotomies and/or long-term ventilatory support.
Unique Aspects of The Rehabilitation and Transitional Care Department
The strength of the department lies in its interdisciplinary approach. All disciplines work cohesively together with patients and families in defining and implementing the plan of care.
Partnership With Families
The strong team approach of the department is strengthened by the ability to develop close partnerships with families. Department staff work closely with families in terms of support, education and assistance in learning to care for their child.
Defining Success
Success of The Rehabilitation and Transitional Care Department is measured by the ability to facilitate the transition to home and community, where patients and their families are able to maximize their independence and achieve successful re-entry into the community.
