How to Refer

Families may self-refer themselves to the Tuberous Sclerosis Clinic at Cincinnati Children's.

Referrals to the Tuberous Sclerosis Clinic at Cincinnati Children's Hospital Medical Center can be made one of two ways:

  • A physician may contact us with a referral
  • The family may call to self-refer

Please contact your health insurance provider to obtain prior authorization.

Test Results and Medical Records

If you have test results or medical records that are not from Cincinnati Children's, please mail them to:

It is important that we receive records and test results on your child or family member before the first visit. Also, please let us know what special concerns or needs you or your child may have before your visit.

Getting Insurance Approval

Here are the steps you need to take in order to get your health insurance provider to cover your visit to the Tuberous Sclerosis Clinic at Cincinnati Children's:

  • Call your health insurance provider and explain the visit.
  • Ask your insurance company if Cincinanti Children's Hospital Medical Center is a provider. (We may also be listed as Children's Hospital Medical Center of Cincinnati.)
  • Verify coverage and whether or not a referral from your child's primary care physician is needed.
  • Additional documentation may be needed from other local providers.
  • Contact Patient Financial Counseling if you feel you may need financial assistance, 513-636-8891.

Financial Assistance

The Tuberous Sclerosis Clinic at Cincinnati Children's accepts all forms of insurance and Medicaid.

Financial assistance is also available. Our social worker can discuss this with you at or before your visit, or you can contact Patient Financial Services directly, 513-636-8891.
