My Care Connection: Chronic Condition Clinical Portals
Welcome to the My Care Connection Chronic Condition Clinical Portals at Cincinnati Children's Hospital Medical Center. The portals are designed to improve the quality of patient care.
Each portal includes accurate, up-to-date medical record information, such as:
- Lab results
- Medications
- Procedures
- Admissions
- Demographics
Portals support the ability for patients and parents to manage a child's condition and provide information parents need in order to be active, informed members of the health care team.
A Secure Way for Families and Health Care Teams to Communicate
My Care Connection also provides a secure way for patients and family members to ask non-urgent questions. Questions are answered by doctors, nurses or other care team members within one business day. Both questions and answers can only be viewed on My Care Connection.
How to Sign Up
Each Chronic Condition Clinical Portal is a secure web site. You need to complete an authorization form to obtain portal access. The authorization form can be downloaded from each portal log in page. Or you can also request an authorization form from clinic staff during your next visit.
The portals are currently available for children with liver transplant, juvenile rheumatoid arthritis, diabetes, cystic fibrosis and kidney transplant conditions.
