Welcome to the My Care Connection Chronic Condition Clinical Portals at Cincinnati Children's Hospital Medical Center. The portals are designed to improve the quality of patient care.

Each patient portal includes accurate, up-to-date medical record information, such as:

• Lab results
• Medications
• Procedures
• Visit History

Portals support the ability for patients and parents to manage a child's condition and provide information parents need in order to be active, informed members of the health care team.

A Secure Way for Families and Health Care Teams to Communicate

My Care Connection also provides a secure way for patients and family members to ask non-urgent questions. Questions are answered by doctors, nurses or other care team members within one business day. Both questions and answers can only be viewed on My Care Connection.

How to Sign Up

Each Chronic Condition Clinical Portal is a secure web site. To sign up, simply enroll online. All online enrollment requests are reviewed within three business days. Or, during your next clinic visit, ask a member of your child's health care team about getting a temporary username and password to use when enrolling online. This will give you immediate access to My Care Connection once you complete the online enrollment process.

You can also complete a paper access form and return it to the appropriate clinic. Access forms can be downloaded from each portal log in page.

The authorization form can be downloaded from each portal log in page. You can also download an authorization form by choosing from the list below:

You can also request an authorization form from clinic staff during your next visit.

Giving Access to Other Family Members

You can grant access to additional family members if:

• You are a parent or legal guardian with an active portal account.
• You are a patient who is over 18 and have your own portal account.

Do not simply share your user name and password with other members of your family. It is important that each person with access to My Care Connection have his/her own unique user ID and password.

To give permission for someone to access My Care Connection, you simply need to:

• Complete a My Care Connection: Authorization for Use and/or Disclosure of Protected Health Information form. Then, mail or fax the form to the appropriate Cincinnati Children’s clinic.
• Once permission has been granted, the family member needs to complete the Patient and Family Request for My Care Connection Online Patient System form. Then, just mail or fax this form to the same clinic.

The Chronic Condition Clinical Portals are currently available for children with liver transplant, juvenile rheumatoid arthritis, diabetes, cystic fibrosis and kidney transplant conditions.
