Fig A:  A conceptual model based on focus groups and interviews with parents and caregivers identified key elements of the family’s experience with the hospital-to-home transition. Within 30 days of a child’s discharge from the hospital, family members identified a range of transitional challenges, including mental exhaustion, information overload, and lack of emotional readiness or confidence to care for a sick child.
