Welcome
The Center for Innovation in Chronic Disease Care (the Center) at Cincinnati Children's Hospital Medical Center was created to improve the outcomes for children and adolescents with chronic illnesses by accelerating the development of effective care delivery methods. With this goal in mind, we have brought together scientists and physicians from eight different academic Divisions and six different disciplines to foster change and facilitate experimentation, as we focus on the needs of patients and their families.
Our aim is to identify and understand the barriers to optimize clinical outcomes and experiences among adolescents with asthma, and we have created the Asthma Innovation Lab to use new knowledge to improve clinical outcomes and experiences. As part of this objective, we are developing improved and innovative care methods, and we will apply successful new care methods to other chronic patient populations.
The Center is passionate about improving outcomes through change and creating the future of chronic care management for patients. The interventions and innovative methods being explored and trialed as part of the Center involve care delivery, education and training, spiritual coping, health-related quality of life, shared decision-making, advanced technology, and self-management/adherence.
