Severe BPD Continues to Drive Long-Term Respiratory Morbidity
Bronchopulmonary dysplasia (BPD) remains one of the most significant complications of extreme prematurity. Infants with severe disease often require prolonged respiratory support and face increased risks of chronic lung disease, pulmonary hypertension, recurrent hospitalizations and long-term cardiopulmonary morbidity.
Despite advances in neonatal care, treatment options remain limited. Current management strategies focus on respiratory support, symptom management and prevention of complications, with few therapies directly addressing the developmental lung injury underlying disease progression.
Cincinnati Children’s Research Identifies a Lung Repair Pathway Relevant to BPD
Investigators at Cincinnati Children’s reported findings in Cell Stem Cell that provide new insight into biological mechanisms involved in lung repair and regeneration.
The study identified disruptions in a pathway associated with normal lung development and repair in BPD. In preclinical models, restoring activity within the pathway improved features of lung development that are impaired in the disease.
While the findings remain investigational, they advance understanding of the mechanisms that contribute to chronic lung injury and identify a potential therapeutic target for future study. The research also reflects a broader effort to develop therapies that address underlying lung injury rather than focusing solely on management of long-term consequences.
Consider Referral for Infants with Severe BPD and Evolving Pulmonary Vascular Disease

- Severe BPD requiring prolonged respiratory support
- Persistent oxygen requirements beyond expected recovery
- Suspected or confirmed pulmonary hypertension
- Complex respiratory sequelae related to extreme prematurity
- Ongoing concerns regarding pulmonary growth and long-term lung function

Looking Ahead: Regenerative Therapies Could Change the Future of BPD Care
Although regenerative therapies for BPD remain under investigation, the study highlights a promising direction for future therapeutic development. By improving understanding of biological pathways involved in lung repair, the research contributes to ongoing efforts to develop treatments that target the underlying causes of chronic lung disease rather than its consequences alone.
When potential therapies move closer to clinical translation, continued collaboration among neonatologists, pulmonologists, cardiologists and community providers will be essential to improving outcomes for infants with severe BPD.
