Pediatric and Adolescent Gynecology Rotation for Ob/Gyn Residents

The Pediatric and Adolescent Gynecology resident rotation at Cincinnati Children’s offers OB/GYN residents focused exposure to the care of pediatric and adolescent patients with gynecologic needs.

This four-week clinical rotation provides experience across outpatient, inpatient, and operating room settings. Residents work alongside pediatric and adolescent gynecology providers and multidisciplinary teams that deliver comprehensive care, including the management of complex reproductive and gynecologic conditions in children and adolescents.

Qualifications / Requirements

Applicants must meet the following requirements:

  • OB/GYN residents (PGY-3 or above) in good standing with their home program
  • Professional liability insurance provided through the home institution
  • Minimum of three months’ notice prior to the rotation start date
  • International trainees must have approval through the Educational Commission for Foreign Medical Graduates (ECFMG)

Duration

  • Rotation length: Four weeks (minimum three weeks with prior approval)

Curriculum

Residents participate in hands-on clinical care across multiple settings, including ambulatory clinics, inpatient services, and the operating room.

Goals / Objectives

The Pediatric and Adolescent Gynecology resident rotation is designed to build clinical knowledge, technical skills, and communication strategies needed to care for children and adolescents with gynecologic concerns. By the end of the rotation, residents will be able to complete the following:

  • Recognize, evaluate, and manage common pediatric and adolescent gynecologic conditions, including:
    • Vulvovaginitis, labial adhesions, vulvar ulcers, prepubertal vaginal bleeding, and lichen sclerosus
    • Abnormal uterine bleeding, dysmenorrhea, pelvic pain, ovarian cysts, PCOS, and pubertal disorders
  • Develop appropriate differential diagnoses for:
    • Primary and secondary amenorrhea
    • Acute heavy menstrual bleeding
    • Pelvic pain and abnormal bleeding in adolescents
  • Describe normal menstrual patterns in adolescents and provide developmentally appropriate education to patients and families
  • Identify indications for imaging, medical management, and surgical intervention, including laparoscopy when appropriate
  • Identify medical conditions where therapeutic amenorrhea may be indicated and counsel families on treatment options
  • Recognize conditions related to sexual differentiation
  • Describe the physical and psychosocial characteristics of early, middle, and late adolescence
  • Counsel adolescents on contraception, including:
    • Benefits, risks, side effects, and contraindications of available methods
    • Emergency contraception eligibility, prescribing, and follow-up
    • Correct use of chosen contraceptive methods and management of common issues
  • Structure gynecologic visits that balance confidentiality, patient autonomy, and family involvement
  • Determine when pelvic examinations are indicated in pediatric and adolescent patients
  • Perform gynecologic examinations in prepubertal children
  • Observe and participate in first pelvic examinations with emphasis on patient-centered communication and trauma-informed care
  • Perform or assist with procedures such as:
    • Vaginoscopy
    • Laparoscopy in pediatric and adolescent patients
    • Diagnostic laparoscopy for endometriosis
    • Laparoscopic cystectomy
    • Cystectomy via mini-laparotomy
    • Hymenal and vaginal septum resection 
    • Assessment of mullerian structures via vaginoscopy, cystoscopy, laparoscopy
    • Laparoscopic oophorectomy for ovarian tissue cryopreservation 
    • Collaborative cases with Pediatric Surgery and Urology in patients with complex anorectal or cloaca malformations
       
  • Access current, evidence-based educational resources to support patient counseling and clinical decision-making

Residents are encouraged to actively review rotation objectives so that rotation can be tailored to the individual resident's experience. Requests made less than three months prior are considered on a case-by-case basis.
