Appointments at Liberty Primary Care Location

Our Centerville Location, including Primary Care, is closed temporarily due to a construction issue that we need to address. The location will likely remain closed for several months for construction.
 
Dr. Johannes, nurses and other staff are temporarily sharing space with Liberty Primary Care. Our phone number 937-535-7880 and hours remain unchanged.
